Skilled Nursing Services are provided in a variety of settings, including inpatient and outpatient facilities, skilled nursing facilities, and in the home. These services are provided by licensed nursing personnel (i.e., RN or LPN) on an intermittent or hourly basis.

Wake Up/Tuck In

Physician’s Choice offers various wake-up and tuck-in services for those who need just a little extra assistance at the beginning and end of each day. Our wake-up/tuck-in clients get the assistance they need to stay at home without feeling completely dependent on a full time, around-the-clock caregiver.

We provide physical therapy for seniors recovering from injury or illness and for those experiencing chronic pain, physical therapy to help relieve pain and restore physical functions such as flexibility, strength, balance and coordination. Our goal with physical therapy for seniors is to make daily tasks and activities easier. And to make seniors as independent as possible.

Hospice

Physician’s Choice Private Duty provides hospice aid services in conjunction with hospice agency for patients that meet the following criteria. Your plan is to keep the patient at home and your family wants to be there for emotional support. Physician’s Choice will take care of the physical demands of hospice and help with your family’s emotional support. We are licensed medication aids and operate under the direction of a registered nurse.

Remote Monitoring

Call us about this new service. Our nurses keep round-the-clock watch on motion and humidity sensors in the living rooms, bedrooms and bathrooms of elderly patients in their own homes. We usesophisticated sensors, computerized pattern recognition and human responders, to detect and head off health threats to your elderly family members, without being intrusive.  The goal is to save money on medical costs and help seniors feel secure enough to age at home and avoid moving to assisted living or a skilled nursing facility.

Catheter Changes

Physician’s Choice Private Duty can help you with urine catheterization care at home; be it catheter insertion, catheter removal or bladder wash. We know what to do in instances when your senior’s catheter comes out, or they have a temperature of 101° F or higher,or they have foul-smelling urine, or bright blood clots, or abdominal pain.

Light Housekeeping

Light housekeeping is part of our healthcare service that makes life at home safer and more pleasant. Examples include things like watering the plants, dusting of surfaces and knickknacks, as well as picking up newspapers, magazines and other clutter. It includes emptying the trash, vacuuming and sweeping of main walk areas. It may also include organizing drawers, shelves and closets. In a nutshell light housekeeping is completing the specific tasks required to tidy up and keep safe the most common areas a client uses.

Ostomy Care

Physician’s Choice provides ostomy patients knowledgeable and supportive nursing care to help them adjust to life with a stoma. With our one-on-one care and attention, patients receive the help they need to live comfortably at home. A Certified Wound and Ostomy Nurse is available for consultation visits as needed.

Feeding Assist

Physician’s Choice provides feeding assistants trained in feeding techniques, assistance with feeding and hydration, knowing how to respond to challenging behavior and recognizing changes in patients that are inconsistent with their normal behavior and relaying those changes to our supervisory nurse.

Mediset Prefill

Physician’s Choice provides a service for preparing a medication device such as a Mediset to facilitate the self administration of subsequent medications. We do this as long as a physician has prescribed the medication and the medications have been properly compounded, packaged, labeled, and delivered to the our clients.

Hoyers and Sit to Stand Services

Physician’s Choice provides Fall Risk assistance and transfers, including special lifts like Hoyer, Sit to Stand, etc. We can assist with ambulation, range of motion, and prescribed exercise programs as instructed by a professional therapist. We are very familiar with transferring and alignment including equipment wheelchairs, crutches, walkers, and safety devices.

Bathing

Everyday activities can become too difficult for your aging parent or disabled family member. Especially bathing and showering which is a daily essential for good personal hygiene. It may be difficult or unsafe for your elder to do alone because bathtubs and showers can be dangerously slippery place. Physician’t Choice provides the help your family member needs as part of our assisted living in home services.
